What is New York Credentials Verification Application?
The New York Credentials Verification service is a verification of the authenticity of an applicant’s physical therapist or physical therapist assistant educational and licensure records. Verification from a New York State Education Department (NYSED) approved Credentials Verification Organization (CVO) is required by NYSED when an applicant is seeking licensure in New York. FCCPT is an approved CVO for both professions. Upon collection of all required documents FCCPT will submit all education and licensure documentation to NYSED. This service does not include collection of documents from U.S. education or licensure institutions. The applicant must contact NYSED for instructions on sending these documents.

New York Credentials Verification Requirements
** To apply for the New York Verification Service you must meet the following requirement,
** A foreign-educated physical therapist or physical therapist assistant seeking a license to practice in New York State.

** Review the Application Fee Schedule, and be prepared to make payment at time of application.
** Applicants who want to apply for a license to practice in New York must also complete the NYSED Application for Licensure, and other forms that may be required, and follow instructions for submission to the NYSED, not FCCPT. This application, additional forms, and instructions for completion are available on the NYSED website.
Document Submission Guidelines:
** Obtaining Documents From Schools & Licensure/Regulatory Authorities
** Send to all appropriate institutions.
** New York Request for Academic Credentials Verification (page one to be filled out by applicant, page two to be completed by registrar or principal of academic institution).
** School sends transcript, transcript of hours, mark sheets, certificate of clinical hours and experiences and syllabus with the form.

Document Submission By Applicant:
** New York Credentials Verification Attestation mailed to FCCPT.
** Physical therapy education diploma(s), degree(s), or certificate(s) to FCCPT, which have been notarized as a true copy of the original.
Note:
** FCCPT will accept attested copies of diplomas, degrees, and/or certificates from the issuing institution, if attested by the school registrar.
** If no foreign licenses are held, you must complete and mail or email the No License Form.
** Download all required forms.
Payment For New York Credentials Verification
The Payment For New York Credentials Verification are given below,
** FCCPT accepts Mastercard and Visa only.
** Please have your payment ready at the time of application.
** If a payment is not made at the time of application, you will have to restart the application.
** We do not create a file or save information from incomplete applications.
Service Timeline For New York Credentials Verification
** FCCPT allows twelve (12) months from the time of application to receive all required documents for a selected service.
** If all documents are not received within twelve (12) months of the application date, FCCPT will close the service, and you must apply for Reactivation to resume the service.
** The following timeframes are for services in review (paid-in-full and all required documents have been received); these timeframes do not include the time necessary to request and receive documents from their appropriate source(s).
Important:The following time frames are all approximate and may take more or less time depending on circumstances.
** Comprehensive Credentials Review (Type 1 Review), Educational Credentials Review, and Physical Therapist Assistant Educational Equivalency Review (PTA-EER) – Approximately twelve (12) weeks
** Re-activation – Approximately twelve (12) weeks
** New York Credentials Verification – Approximately two (2) weeks
** Re-Evaluation (of Type 1 Review, Educational Credentials Review, or PTA-EER) – Approximately six (6) weeks
** Type 1 Review after a previously completed Educational Credentials Review – Approximately twelve (12) weeks
** CWT Version Update – Approximately twelve (12) weeks
** Duplicate Report – Approximately three (3) weeks
** Type 1 Renewal – Approximately three (3) weeks
** USCIS Status Letter:
** Status Letter – Approximately three (3) weeks
** Expedited Status Letter – Approximately one (1) week
** Certified Documents – Approximately three (3) weeks
Withdrawing an application:
** You may withdraw an application within five (5) business days of submission of the application.
** To withdraw, you must download, complete, and upload the Cancellation of Service Form to FCCPT.
** FCCPT’s mailing address can be found on our Contact Us page.
** Refunds are completed within thirty (30) days.
** There will be no refund for a withdrawal after five (5) business days.
** If you subsequently reapply for the service, the full fee will be required at that time.
Changing A Service
** You may request a change of your service within five (5) business days of submission of the application.
** To change your service, you must download, complete, and upload the Change of Service Form to FCCPT.
** There is a service fee for changing a service after five (5) business days from submission of your application.

If you currently have a service in-progress, or have had a previous service with FCCPT, you should use your existing file number to view your progress or apply for a new service. If you cannot remember your file number or are unable to access your file for any reason, do not open a new file.
